Abstract:
Mide ışınlamasında hedef hacimdeki yapı farklılıkları ve hedef hacmin yakın komşuluğundaki kritik organlar sebebiyle son derece karmaşık bir tedavi planı ortaya çıkmaktadır. Mide radyoterapisinde hedef hacmin homojen bir şekilde ışınlanması amaçlanırken; sol böbrek, sağ böbrek ve karaciğer gibi sağlıklı organların mümkün olduğunca az doz almasına çalışılır. Bu sebeple mide radyoterapisinde farklı tedavi tekniklerine başvurulur. Bu kullanılan tekniklerde midenin aldığı doz, iyonizan radyasyonun sağlam dokular üzerindeki etkisi sebebiyle önemlidir. Bu tez çalışmasında konvansiyonel ışınlama (AP-PA), üç boyutlu konformal (3BKRT) ve alan içinde alan (FIF) ışınlama tekniklerinin kullanıldığı mide ışınlamalarında midenin ve kritik organların aldığı dozun araştırılması amaçlanmıştır. Bu çalışma, Şanlıurfa Mehmet Akif İnan Eğitim ve Araştırma Hastanesi Radyasyon Onkolojisi Kliniğinde XIO Version 4.8 tedavi planlama sistemi kullanılarak yapılmıştır. Çalışmada dokuz mide kanseri vakasına konvansiyonel, konformal ve FIF tekniklerine göre planlamalar yapılmıştır. Hedef hacim ve sağlıklı organ dozları tedavi planlama sisteminde elde edilen doz-hacim histogramları ile belirlenmiştir. Sonuç olarak, mide ışınlanmasında kritik organları korurken, hedef hacimde homojen bir doz dağılımı sağlayan Üç Boyutlu Konformal Radyoterapi (3BKRT) ve Alan içinde Alan (FIF) tekniklerinin kullanılmasının daha uygun olduğu sonucuna varılmıştır.
Gastric irradiation becomes quite complicated due to the structural differences in the target volume and due to critical organs at close proximity to the irradiation field. In gastric radiotherapy, while the aim is to homogeneously irradiate the target volume, the dose to the healthy organs such as left kidney, right kidney and liver should be kept at as low values as possible. Therefore, different treatment techniques are used in gastric radiotherapy. In these techniques, the dose received by the stomach is important because of the effect of ionizing radiation on the healthy tissues. The aim of this thesis study is to investigate the doses in stomach and critical organs during gastric irradiations using conventional (AP-PA), conformal (3DCRT) and field-in-field (FIF) irradiation techniques. This study was performed at the Radiation Oncology Clinic of Şanlıurfa Mehmet Akif Inan Training and Research Hospital by using XIO Version 4.8 treatment planning system. In this study, radiotherapy plannings of nine gastric cancer cases were performed according to conventional, 3DCRT and field-in-field techniques. Doses of target volume as well as critical organ doses were determined using dose-volume histograms obtained from the treatment planning system. It is concluded that in gastric irradiation the use of the 3DCRT and the field-in-field techniques are more appropriate both for protecting the target volume and the critical organs at the same time achieving more homogeneous dose distribution for the target volume.
