Yenidoğan yoğun bakım ünitelerinde çalışan hemşirelerin gelişimsel bakıma yönelik bilgilerinin incelenmesi / Analysis of the knowledge levels of the nurses working in the neonatal intensive care units on developmental care

Abstract

Çalışma yenidoğan yoğun bakım ünitesinde çalışan hemşirelerin yenidoğanın bireyselleştirilmiş destekleyici gelişimsel bakımına yönelik bilgi ve uygulamalarını belirlemek amacıyla tanımlayıcı olarak planlanmıştır. Araştırma Şanlıurfa ilinde Yenidoğan Yoğun Bakım Ünitesi bulunan 1 üniversite hastanesi, 2 kamu hastanesi, 4 özel hastanede Eylül 2015-Ocak 2016 tarihleri arasında yapılmıştır.Hemşirelerin %59,8'inin 18-25 yaş arasında olduğu, %49.5'i lisans mezunu ve %63,2'si bir yıldan uzun süredir yenidoğan hemşiresi olarak çalışmaktadır. Hemşirelerin %73,5'i gelişimsel bakım konusunda eğitim aldığını, %27,9'unun eğitimi yeterli bulduğu ve %80,9'u çalıştıkları ünitede gelişimsel bakıma yönelik uygulama yaptıklarını bildirmiştir. Çalışmanın sonucunda araştırmaya katılan hemşirelerin eğitim durumlarına göre gelişimsel bakım uygulama durumlarının karşılaştırılması amacıyla yapılan analizde, bebeğe gereksiz dokunmadan kaçınma ve ailenin bakıma katılması arasında istatistiksel olarak anlamlı fark saptanmıştır. Yenidoğanda çalışma yılına göre kuş yuvası tekniği ile yatırılması arasında istatistiksel olarak anlamlı fark saptanmıştır. Hemşirelerin gelişimsel bakım eğitimi alma durumlarına göre, düşük ses düzeyinin sağlanması, akşam ve gece ünitede ışıkların kapatılması ve azaltılması uygulaması, küvözün üzerinin örtülmesi, bebeğin stres belirtileri yönünden değerlendirilmesi ve kuş yuvası tekniği ile yatırılması arasında istatistiksel olarak anlamlı fark saptanmıştır. Hemşirelerin çalıştıkları kurum türü ile kanguru bakımı uygulanması ve bebeğin alınmasından itibaren bakıma ailenin katılım durumları arasında istatistiksel olarak anlamlı fark saptanmıştır.Gelişimsel bakım eğitimlerinin sürdürülmesinin yenidoğan ve çocuk sağlığına önemli katkılar sağlayacağı düşünülmektedir. The study was designed as a descriptive study to determine the knowledge level and practices of the nurses working in the neonatal intensive care units on the individualized neonatal supportive developmental care. The study was conducted between September 2015 and January 2016 in 1 university hospital, 2 public hospitals and 4 private hospitals having a neonatal intensive care unit in Şanlıurfa, Turkey.59.8% of the nurses were between the ages of 18-25, 49.5% of them had a bachelor's degree and 63.2% of them had been working as newborn nurses for more than 1 year at the time of the study. According to their statements, 73.5% of the nurses were educated about developmental care, 27.9% of them found thought that education was sufficient, 80.9% of them performed applications for developmental care in the unit which they worked. As a result of the study, it was found that there was no statistically significant difference between keeping away from unnecessary physical contact with infants and parental involvement in care, in the analysis of the developmental care application status of the nurses participating in the study. A statistically significant difference was found between putting the infant to bed with the bird nest technique according to the year of the study in the infants. According to the developmental care education status of the nurses, a statistically significant difference was found between maintaining a low sound level, turning off and reducing the lights in the evening and night, covering the incubator, evaluating the infant according to the stress symptoms and putting the infant to bed with the bird nest technique. There was a statistically significant difference between the type of institution that the nurses were working and kangaroo care and the participation status of the family since the beginning of the care. It is thought that the maintenance of developmental maintenance training will provide significant contributions to the health of neonate and children.
