Sağlık hizmetlerinin kamusal niteliği sağlık harcamalarının gelişimi: Seçili ülkeler ve Türkiye üzerine karşılaştırmalı bir inceleme / Public quality of health services and improvement of health expenditures: A comparative study on selected countries and Turkey

Abstract

Sağlık, kişilerin ruhsal, fiziksel ve sosyal yönden iyi olması olarak tanımlanmaktadır. Bireylerin sağlıklı olması toplumu da etkilemektedir. Sağlıklı kişilerin toplumsal yararı şüphesiz çok yüksektir. Tersi durumda ise, meydana gelen zararların boyutları ve maliyetinin tahmin edilmesi dahi çok zordur. Bu maliyetlerin karşılanması için yapılan harcamaların tümü sağlık harcamalarının konusunu oluşturur. Bu çalışmanın amacı; sağlık sistemleri ve sağlık harcamalarını inceleyerek Türkiye ile bazı gelişmiş ülkeler arasında karşılaştırma yapmaktır. Çalışmada; sağlık hizmetlerinin tanımı, özellikleri, sınıflandırılması, sağlık harcamalarının artış nedenleri ile finansman sistemleri açıklanmıştır. Ayrıca sağlık harcamalarının OECD ülkeleri içinde GSYİH'ye oranı ve kişi başına düşen sağlık harcamasının dağılımı tablolar aracılığıyla anlatılmaya çalışılmıştır. Türkiye'de sağlık hizmetlerinin tarihsel gelişimi, harcamaların finansmanı ile sağlık hizmetlerinin sunumu hakkında açıklamalara yer verilmiştir. Yapılan incelemeler ve rakamsal veriler ışığında; Türkiye'nin ve ele alınan seçili gelişmiş ülkeler arasında önemli ölçüde farklar olduğu tespit edilmiştir. Türkiye'de sağlık hizmetlerinin sunumunda bazı eksiklikler olduğu tespit edilmiştir. Health is defined as the well-being of people in the spiritual, physical and social aspects. The health of individuals also affects society. The social benefits of healthy people are undoubtedly very high. Otherwise, it is very difficult to estimate the size and cost of the resulting damage. All costs incurred to cover these costs constitute the subject of health expenditures. The purpose of this study is to make some comparisons between some developed countries and Turkey by examining health systems and health care spending. In this study, definition of health services, their characteristics and classification, reasons of health expenditure increases and financing systems are explained. In addition, the ratio of health expenditures to GDP in OECD countries and the distribution of health expenditure per capita have been attempted to be explained through tables. Explanations about the historical development of health services in Turkey, the financing of expenditures and the provision of health services have been given in the study. In the light of the examinations and numerical data, it was found that there were significant differences between Turkey and the developed countries. It was also found that there were some deficiencies in the provision of health services in Turkey.
