Abstract:
Kardiyopulmoner Bypass (CPB), kalbin pompalama ve akciğerlerin gaz alışverişi işlevinin geçici bir süre ile kalp-akciğer makinesi tarafından sağlanmasına denir. CPB, kalpten gelen venöz kandan karbondioksit (CO2) alıp oksijen ilave ederek geniş bir artere geri gönderen bir tekniktir. Sonuç olarak tam olarak sağlandığında, ekstra korporeal dolaşım, hem sirkülasyonu, hem de ventilasyonu sağlar. Kardiyopulmoner bypass sırasında miyokard belli bir süre içinde perfüze edilememekte, dolayısıyla miyokard global iskemik hasara maruz kalmaktadır. Miyokard?da oluşan bu hasara temelde reperfüzyon sırasında oluşan serbest oksijen radikalleri ve oksidatif stres neden olmaktadır. Bu çalışmaya çeşitli sebeblerden dolayı kardiyopulmoner bypass cerrahisi ile ameliyat olan toplam 30 hasta dâhil edildi. Bu hastalardan kardiyopulmoner bypass öncesinde, pompaya giriş sırasında, pompadan çıkış sırasında ve ameliyat sonrası olmak üzere toplam olarak 4 tüp kan alınarak bir çalışma grubu oluşturuldu. Daha sonra toplanan kanlardan akciğer ventilasyonuna bağlı olarak ventilatörle %10 ve tamamen durdurulan hastaların total oksidatif stres (TOS),total antioksidan kapasite (TAK), oksidatif stres indeksi (OSİ=total Oksidatif Stress (TOS) / total antioksidan kapasite (TAK) şeklinde bölünerek) üzerindeki etkileri çalışıldı. Bu çalışma sonucu: kardiyopulmoner bypass sırasında, oksidatif stres indeksinin akciğer ventilasyonu ile azaldığı görülmüştür (p<0.05). Cardiopulmonary Bypass (CPB) is defined as providing the gas Exchange function of the lungs and the pumping function of the heart by the heart-lung machine for a transient period. CBP is a method which adds oxygen into the venous blood originated from the heart by eliminating CO2 and sending it back in to a large artery. Consequently, if the system is completely fixed, the extracorporeal circulation provides both the circulation and the ventilation. Myocardium can not be perfused during cardiopulmonary bypass. Consequently, myocardium is exposed to global ischemia. Free oxygen radicals and oxidative stress which occured during reperfusion ,cause myocardium damage. This study evaluated 30 patients who underwent CABG operation. Blood samples are taken fram the patients before cardiopulmonary bypass, at the time of initiation CPB, at the timeof weaning of CPB and after operation. Afterward the blood samples are analysed, that lung ventilation effect on TOS, TAK, oksidatif stres index (TOS/TAK) with %10 ventilationan and without ventilation.
