Esansiyel hipertansiyon hastalarında oksidan ve antioksidan statü değerleri ile eritrosit ve trombosit indekslerinin araştırılması / Investigation of total oxidant and antioxidant status, erythrocyte and platelet indices in essential hypertension patients

Abstract

Giriş: Hipertansiyon, gelişmiş ülkeler başta olmak üzere tüm dünyada gittikçe artan önemli bir halk sağlığı sorunudur. Dünyada yaklaşık olarak 1 milyar kişide hipertansiyon olduğu ve yılda 7,1 milyon kişinin hipertansiyona bağlı olarak öldüğü ileri sürülmektedir. Bu nedenle, hipertansiyon gelişim sürecinde oksidan ve antioksidan değerlerin etkileneceği, eritrosit (MCV, MHC, MCHC ve RDW) ve özellikle trombosit indeksleri (PLT, MPV, PCT ve PDW)'nin hastalığın etiyolojisinde etkili olabileceği düşünülmektedir. Bu çalışma, hipertansiyon hastalarında lipit profili değerleri, eritrosit ve trombosit indeksleri, oksidan (TOS, OSI) ve antioksidan statü (TAS) değerleri ile paraoksanaz 1 (PON1) ve arilesteraz (ARE) aktivitelerini araştırmak amacı ile yürütülmüştür. Materyal ve Metod: Çalışmada Şanlıurfa Eğitim ve Araştırma Hastanesinde hipertansiyon teşhisi konarak ilgili kliniğe yatırılan hastalardan rutin analizler için alınan ve gerekli analizler yapıldıktan sonra atılmak üzere ayrılan kan örnekleri, atılmadan önce alınmış ve gerekli analizler için kullanılmıştır. Araştırmada 46 hipertansiyon hastası ile 39 sağlıklı kontrol grubunu oluşturan bireylerden kan örnekleri alınmıştır. Plazma örneklerinde lipit profili ve TAS, TOS, OSI değerleri ile ARE ve PON1 aktiviteleri ölçüldü. Ayıca tam kanda eritrosit ve trombosit indeksi değerleri belirlendi. Elde edilen ham veriler bilgisayarda istatistik programı ile analiz edilmiştir. Bulgular: Elde edilen bulgularda hipertansiyon grubunda; trigliserit, kolesterol, LDLkolesterol ve VLDL düzeyleri ile TAS değerlerinin azaldığı tespit edilmiştir. TOS ve OSİ değerleri ile PON1 ve ARE enzim aktiviteleri bakımından kontrol grubu ile hipertansiyon grubu arasında istatistiksel olarak anlamlı farklılıklar bulunmuştur. Sonuç: Hipertansiyon hastalarında antioksidan savunmanın zayıfladığı, ARE ve PON1 enzim aktiviteleri ile lipit değerlerinin etkilendiği gözlemlenmiştir. It has been put forward that approximately one billion people in the world have hypertension and annually 7,1 million people die as a result of hypertension. Therefore, it is thought that in progress of this illness, oxidant and antioxidant values may be affected, erythrocyte (MCV, MHC, MCHC and RDW) and especially thrombocyte indices (PLT, MPV, PCT ve PDW) may have a role in the etiology of disease. In this study planned, lipid profile, erythrocyte and thrombocyte indeices, oxidant and antioxidant status values and activities of Paraoxonase 1 and arylesterase were studied and the effects of these parameters in the etiology and prognosis of the disease were evaluated. Materials and Methods:In the study, blood samples which were taken from the patients hospitalized with the diagnosis of hypertension in Şanlıurfa Training and Research Hospital during their routine analysis and then centrifuged to seperate blood celis. In the study, samples were taken from 46 patients with hypertension and 39 health people in control group. In plasma samples, lipid profile, total antioxidant status (TAS), total oxidant Status (TOS), oxidative stres index (OSI) values and activities of arylesterase (ARE) and paraoxonase were measured. Obtained raw data was analyzed using a statistical programme SPSS for windows. Results: The obtained findings in the hypertension group; triglycerides, cholesterol, LDL-cholesterol VLDL levels and TAS values are reduced.TOS, OSİ values and PON1 and ARE enzyme activities were statistically different between the control group and hypertension group. Conclusion: As a results antioxidant defense was weakened in the process of hypertension, ARE and PON1 enzyme activities and lipid values have also been affected.
