Investigation of total oxidant and antioxidant status and indices of platelet and erythrocyte in second and third degree burn patients / 2. ve 3. derece yanıklı hastalarda total oksidan ve antioksidan statü ile eritrosit ve trombosit indekslerinin araştırılması

Abstract

Amaç: 2. ve 3. derece yanıklı hastlarda ilk 24 saat içerisinde plazma TAS, TOS, OSİ, paraoksonaz, arilesteraz aktiviteleri ile eritrosit (RBC, HGB, HCT, MCV, MCH, MCHC, RDW ) ve trombosit (PLT, MPV, PCT, PDW) indekslerinin plazmadaki değişimlerini belirlemek. Diğer bir amacımız yanıklı hastalarda lökosit değerlerini belirlemek. Yöntem: Çalışmamız 2. ve 3. derece yanık yaralanması olan 42 hasta ve 38 sağlıklı grup ile yürütülmüştür. Yanık grubu acil servise tedavi amaçlı başvuran ikinci ve üçüncü derece yanığı olan hastalar üzerinden seçildi. Çalışmamıza ikinci ve üçüncü derece yanık yaralanması olan hastaların ilk 24 saatlik kan örnekleri dahil edildi. Sağlıklı grup çalışmayı etkileyecek özelliği ( metabolik hastalık, sigara ve alkol kullanımı, böbrek rahatsızlığı vs. ) olmayan kişiler arasından seçildi. Kan- plazma örnekleri 4000 devirde santrifüj edildikten sonra -80?de muhafaza edildi. TAS, TOS, OSI düzeyleri, platelet ve eritrosit indeksi, PON-1 paraoksonaz ve arilesteraz aktiviteleri deneklerden alınan kan örneklerinden ölçüldü. Bulgu: TAS, TOS, OSİ serum değerlerinde hasta grupta sağlıklı gruba göre anlamlı bir farklılık yoktu (p?0.05). PON-1 paraoksonaz aktivitesinde anlamlı bir farklılık görülmezken, PON-1 arilesteraz aktivitesinde yanık hasta grubunda sağlıklı gruba göre anlamlı oranda düşük bulundu (p?0.05). Eritrosit indeksleri arasında hasta ve sağlıklı grup arasında istatiksel bir fark yoktu. Trombosit indeksleri arasında ise PLT, MPV, PCT değerleri anlamlı olarak yüksek bulunurken(p?0.05) PDW değeri ise hasta grupta anlamlı olarak düşük bulundu(p?0.05). Cinsiyet olarak grubumuz arasında anlamlı bir farklılık yoktu. Lökosit değerleri kontrol grubuna göre anlamlı düzeyde yüksek bulundu. Sonuç: Çalışmamızda 2. ve 3. derece yanıklı hastalarda ilk 24 saat içerisinde TAS, TOS, OSİ değerleri değişmedi, eritrosit indeksi değişmezken trombosit değerlerinde artış görüldü. Anahtar Kelimeler: Yanık, Oksidatif Stres, Eritrosit İndeksi, Trombosit İndeksi, Antioksidanlar Aim: To determine the changes in plasma TAS, TOS, OSI, paraoxonase, arylesterase activities, erythrocyte ( RBC, HGB, HCT, MCV, MCH, MCHC, RDW ) and platelet indices in the first 24 hours in second and third degree burn patients. Our another goal is to determine the values of leukocytes on the burned patients. Methods: Our study was performed in 42 patients with second and third degree burn and 38 healthy groups. The burn group was selected from patients who admitted to emergency service for the treatment of second- and third-degree burns. The first 24 hour blood samples of the patients with second and third-degree burn injury were included in our study. The healthy group was selected from people who didn?t have any property ( metabolic disease, smoking, alcohol , kidney disease, etc ) that affects the study. Blood-plasma samples were stored at - 80 after centrifugation at 4000 rpm. TAS, TOS, OSI levels, platelet and erythrocyte index, PON-1 paraoxonase and arylesterase activities were measured in the patient?s serum samples. Result: There were no significant difference in the serum level of TAS, TOS, OSI between patients and healthy groups (p?0.05 ). PON-1 arylesterase activities in patients with burn injury were significantly lower than healthy groups but there were no significiant difference PON-1 paraoxonase activities (p?0.05). There was no statistical evidence of difference in the serum levels of erythrocyte indices between burn patients and healthy groups. PLT, MPV, PCT levels of platelet indices in the patient group were significantly higher than healthy groups (p?0.05), PDW levels in the patient group were significantly lower than the healthy goups (p?0.05). There was no significant difference between the gender in our groups. Leukocyte counts were found significantly high according to control group. Conclusion: In our study, TAS, TOS, OSI levels and erythrocyte index did not change, platelet count increased in the first 24 hours in the patients with second and third degree burns.
