Abstract

Giriş ve Amaç: İntihar davranışı son yirmi yıldır, kültürel farklılıklar olmakla birlikte, hastalığa bağlı olmayan ölümlerin çoğunluğuna neden olan, dünya çapında önemli bir halk sağlığı sorunudur. İntihar girişim sıklığının önemli ölçüde artması ile birlikte, sağlık çalışanlarının intihar girişiminde bulunan olguların ihtiyacını tanıma ve bu ihtiyaçlara cevap verebilmedeki başarısızlığı tekrar eden intihar girişimlerinin nedenlerinden biri olabileceği konusunda bulgular vardır. İntihar girişiminde bulunan bireylere doğru sağlık bakımı vermek erken ölümleri önlemenin en etkili yöntemidir. Sağlık bakımının da sağlık çalışanlarının tutumu ile ilişkili olduğu bildirilmektedir. Bu çalışmanın amacı; sağlık çalışanlarının intihar girişiminde bulunmuş bireylere yönelik tutumunu ölçebilmek amacı ile bir ölçme aracı geliştirmektir. Gereç ve Yöntem: Literatürdeki benzer çalışmalar ve uzman görüşleri sonrasında 58 madde içeren bir anket formu geliştirilmiştir. Pilot çalışma, örneklemin dışında olan 21 sağlık çalışanı (doktor, hemşire ve acil tıp teknisyeni) ile yapılmıştır. Esas uygulama daha sonra Şanlıurfa ilindeki 14 hastanede çalışan 230 doktor, hemşire ve acil tıp teknisyeni ile yürütülmüştür. Yüzey geçerliliği için uzman görüşlerinden faydalanılmıştır. Yapı geçerliliğini ölçmek amacı ile faktör analizi yöntemi kullanılmıştır. Ölçeğin farklı zamanlarda aynı ölçümü yapıp yapamadığı da güvenirlik analizlerinden test-tekrar test yöntemi ile değerlendirilmiştir. Bulgular: Ön analizler sonrasında 58 maddenin 30 maddesi ölçek dışında tutulmuş ve 28 madde kalmıştır. Bu 28 maddenin faktör analizi yapıldığında toplam varyansın %58.5' ini içeren altı alt faktörden oluştuğu görülmüştür. ?İntihar Girişimine Yönelik Tutum Ölçeği (İGYTÖ)? nin güvenirlik analizleri sonucunda Cronbach alfa güvenirlik katsayısı 0.84 bulunmuş, test tekrar test analizi de ölçeğin güvenilir olduğunu göstermiştir. Sonuç: Geliştirilen bu ölçeğin güvenilir ve geçerli olduğu, bundan sonraki benzer çalışmalarda kullanılabileceği ve sağlık bakım kalitesinin arttırılmasına yönelik çalışmalara katkıda bulunabileceği düşünülmektedir. Anahtar Kelimeler: İntihar girişimi, sağlık personeli, acil servis, tutum, geçerlik, güvenirlik Suiciding in addition to the cultural differences, is an important public health issue all around the world causing lots of deaths, which doesn?t depend on an illness, for the last twenty years.With the significantly increase of the suiciding attemps there are some evidence that the failure of the health care the provider?s recognizing the needs of the suicidal people and comparing the needs of them may be the causes of recurrent suicide attemps. Providing the best health care to the patiens who attemp suicide is the best effective way of preventing the early deaths.It?s reported that health care is related with the health care providers? attitudes.The aim of the study was to develop a measurement in order to measure the attitudes of the health care providers towards patients who have attempted suicide. Method and Material: After all the similar works in the literature and expert views , a questionnaire with 58 items was developed. A pilot study was carried out with a sample of 21 emergency departmant health professionals. Basic application was conducted with 230 doctors, nurses and emergency medical technicians, working in 14 general hospitals in Şanlıurfa. For face validity, experts opinions are used. In order to establish the scale?s construct validity, a factor analysis was performed. a suface validity of the measurement tool was assessed by a group of experts. Reliability of the questionnaire was assessed by test - retest. Results: After the preliminary analysis, 30 items of the 58 total items were out of the questionnaire , there were left 28 . When the factor analysis of the 28 items was done, the six factors yielded accounted for the 58.5 % level of the total variance. The resultant ? Attidues Towards Attemped Suicide İntrument (İGYTÖ ) achieved high internal consistency, with Cronbach?s alpha of 0.84 in test and a = 0.90 in retest assessment. Conclusions: It?s thought that this measurement tool which was developed, is reliable and valid and can be used in similar researches and enable health care professionals to enhance their understanding of their attitudes towards patients who attempt suicide in order to enhance the provision of effective care to them.
