Abstract:
Ekstrakorporeal dolaşım sistemi kalp için sirkülasyonu, akciğerler içinde ventilasyonu sağlayan mekanik bir sistemdir. Kardiyopulmoner bypass sırasında üç faklı zamanda komplaman sistem aktive olur: Uygulanan heparinizasyon ve heparinden sonra nötralizasyon için kullanılan protamin sonrası (heparin-protamin kompleksi), kanın endotel dışı yüzeylerle teması ve iskemik kalbin reperfüzyonundan sonra. Protamin komplaman sistemi aktive etmekte, fibrinolitik ve koagülasyon sisteme etki ederken humoral bir yanıt oluşturmaktadır. Oluşan yanıt sonucunda oksidatif stres artmaktadır. İmmün sistem aktive olması sonucunda ise sistemik inflamatuar yanıt oluşmaktadır. Çalışmamıza kardiyopulmoner bypass cerrahisi ile ameliyat olan toplam 30 hasta dahil edildi. Hastalardan kan örnekleri kardiyopulmoner bypass pompa giriş esnasında ve kardiyopulmoner bypass sonrasında protamin verildikten 10 dakika sonra olmak üzere 2 çalışma grubu oluşturuldu. Bu kanlardan ayrılan plazmalardan toplam Oksidan Status (TOS),Toplam Antioksidan Status (TAS), Oksidatif Stres İndeksi (OSİ=Toplam Oksidan Status (TOS) / Toplam Antioksidan Status (TAS) şeklinde bölünerek) çalışıldı. Bu çalışma sonucu: kardiyopulmoner bypass sırasında, oksidatif stres indeksinin artırdığını göstermektedir. The study was included 30 patients who underwent surgery with cardiopulmonary bypass surgery. the working group was created into two groups : Pump blood samples from patients before cardiopulmonary bypass and after cardiopulmonary bypass in 10 minutes after protamine administration .Bloods separated from plasmas and were studied Total Oxidant Status (TOS), total antioxidant Status (TAS), oxidative stress index (OSI = Total Oxidant Status (TOS) / Total Antioxidant Status (TAS) divided form). During cardiopulmonary bypass as a result of this study, indicate an increased oxidative stress index.
